NURS 439 - Nursing Care of the Individual II: Theory (3 units) Uses critical analyses and clinical reasoning in caring for diverse individuals with complex chronic disease. Focus on safety, especially during care transitions. Explores the nurse’s role on the interprofessional team.

Student Learning Outcomes Upon completion of this course, students will be able to: 1. analyze the relationship between illness chronicity and acute care to develop individualized plans of care. 2. develop an individualized plan to provide continuity of safe care. 3. evaluate client response to plan of care to guide future interventions and needs. 4. describe disease progression to recognize potential complications. 5. develop an individualized teaching plan that includes client and support systems to promote and manage current and future health. 6. explain treatment strategies and symptom management for diverse chronically ill clients receiving palliative care to improve quality of life. 7. adopt a supportive role in a client’s dying and death processes to help ensure a positive closure of life. 8. develop professional nursing practice and self-advocacy strategies.
